Request an Achievement Record (Transcripts)

For a copy of your Diploma or for Transcripts, please complete the form below at the Ministry of Education Website. https://formulaires.education.gouv.qc.ca/dev_ti/412_demande_releves_diplomes_secondaire/en.

• You can leave the field for Permanent Code blank, if you do not know it.  
• Please be aware that transcripts can only be generated for Grade 10 and above. For grade 9 and under, please contact the last school that you attended with us to get a copy of your report cards.
